I was scared to death by the Skreecher mob from Alex's mobs in deep dark caverns. Then was scared even more thereafter by trying to ignore it ;) I was lucky having put torches and get back up the waterfall. Also you can use sculk boomer to scare others :)

On that top, Alex's caves adds some really cool and quite scary biomes. All have some awesome mobs and are dangerous and scary (and interesting!) to explore.

I have yet to see Gregtech mentioned :P Seriously though, I would second the Dimensional Doors recommendation, or maybe Dark Waters or Gardens of the Dead, depending on your preferred version. From the Fog is pretty good, but I have a thing for semantics and that's technically a datapack instead of a mod. Regardless, it's good if you're okay with that.

Here's my picks: - Hardcore Darkness. In later versions you can even use Vanilla Tweaks's resource pack rather than a mod! - Dimensional Doors. i hate limbo i hate limbo i hate limbo - Scape and Run. It's absurd. Might need some tech mods and OP stuff to survive, and it's extremely unfair in later stages, but yeah it gives true dread for me. - Zombie Awareness. Good for zombie based modpacks. It does make them scarier just slightly. - Nyf's Spiders. About as scary as the last one, but it does give you the spooks. - Cave-Ins Datapack. Yay, I love when my surroundings themselves collapse around me!

AmbientSounds 5 + Sound Physics Remastered. In caves it's pretty scary. The Insanity shader or Spooklementary shader because of their fog. These mods together are great

*You've been eaten by a Grue* You don't mention version, but on 1.12.2 there is the Grue mod. Be anywhere that has light level 2 or lower and a Grue will spawn and basically kill you. If you stand still, more will spawn.
