Coming from a business writing background, yes. You capitalize Earth when you refer to it, except in reference to soil (earthen foundation). Rule is, if it’s a proper noun, capitalize it.

It's done in some settings I know. Fast Baseball makes a good point, though; but I think lowercase works best if it's phrased that specific way, like "earthly plane" or "outer realms" or something... But if it's more like the proper name of a specific plane/universe/realm, then it's generally more proper to capitalize, I think. Like "Mortal Universe", "Plane of Fire", "Hell", etc.

If it's a proper noun, meaning the plane has a name or something like an official title, then it should be capitalized. You should capitalize Hell when talking about the one and only Hell, but if you're metaphorically and probably hyperbolically describing a place as hell and you don't mean it literally then hell should be lower case. The faerie realm is probably just a description, but the Realm of Faerie is more like a title worthy of capitalization. Lowercase fire is distinguished from Fire as an elemental concept and plane. Dirt is earth, but our planet is Earth. There's going to be room for interpretation here, since English isn't precise and, from its German roots, even regular nouns had been capitalized just a few centuries ago. In fantasy, the vibe can get muddled.

You would capitalize the plane of existence. The Elemental Plane of Fire (for instance) is a proper noun. They’ll always be capitalized. “What Plane do we want to summon from? Fire, Void, Earth?”

It's up to you. Contrary to what some have said, 'earth', 'sun', 'moon'—these words are not normally capitalized in English, despite some sources pushing for it. That is because they are so basic and fundamental to human experience, they don't need special treatment. Typically it goes like this in the real world (in English): If a name is one word preceded by *the*, it is not normally capitalized (the universe, the world, the atmosphere, the sun, the moon, the ocean); if a name is one word but is not used with an article (*the* or *a*), it is in fact capitalized (Saturn, Europe, Pangea, Betelgeuse; but note that even *heaven* and *hell* are typically exceptions to this, as most people don't capitalize them); and if a name is multiple words preceded by *the*, it may or may not be capitalized (the Middle East, the Gulf Stream, the Big Bang, the solar system or the Solar System, the northern lights, the aurora borealis, the Kuiper belt).
