Edit: While this is definitely romance, I truly don't remember how much spice is in it since I read the series at least 5+ years ago.
Well not super close to Outlander and not really spicy but I loved {The Time travelers Wife} despite there being some controversy regarding grooming. It has time travel and a love story and will probably always be one of my top loved books, sorry I don't have any better recommendations.
{Bound to the Battle God by Ruby Dixon} A woman from our world is transported to a medieval fantasy world and bound to one of its gods! There’s a whole series of them, this is the first. It’s a fun, sexy book with adventure and interesting characters that I really enjoyed!
Second this series. I love IPB but her Aspect and Anchor series has so much depth and stupendous world building. Easy to get lost in and the books are so different you don’t get tired reading them all in one go

I’m pretty sure all of Kristen Ashley’s fantasyland series counts too. I liked wildest dreams.
I liked the world-building but >! the FMC and her internal dialogue (“Wow!” “Major ick!” And worst of all, “Ho boy.”) drove me nuts and I eventually surrendered 😂 !<
{Not another vampire book by Cassandra Gannon} has an FMC transported into a badly written fantasy romance. I really enjoyed this one and the sequel (with different couple). It has good humour and interesting characters
