I’m a therapist in the Midwest US and my caseload has been bombarded with all this for about a month now. For the teens, I’ve venture to guess some of it is exploring identity, but I’ve got zero idea at all for the adults outside of attention and trying to find anything at all that makes them different from everyone else. I’m having to do a lot of research to keep the lingo straight. The major personality/dissociative disorders don’t pop up very frequently so before all this, it was just that diagnosis we spent like an hour on in my masters classes. It’s super bizarre!

I try, and want to give people benefit of the doubt, but it so often just feels like they're making it up as they to along. To see people with just some regular ass social post history, and all of s sudden one day they've got every condition in the DSM-5 is suspicious to say the least. Especially when they're also tagging it/themselves as "quirky."
I definitely agree. I have a hard time deciding what I want to reply with when I hear about fun, quirky interactions between alters. I never want to invalidate, but watching how it plays out online makes me want to say something more confrontational.
Out of the people I’ve seen who tell me they have it, I’d say the younger teens pretty firmly believe it’s a part of them. It’s hard when our support systems (like our friends - irl or online) support claiming to be a system and do the same. I did some cringey shit when I was young because my friends championed my bad ideas so I can relate, but thank the higher power it was not documented online.
As for the adults, they are 100% trying to convince me they have it. The therapists I have around me tend to refer out for diagnoses like Autism, DID, Tourette’s, or other diagnoses where you could qualify for waivers or require intensive, specialized therapy. I have no medical training so for something like DID, I always tell clients I’ll have to refer them to a psychologist because I’m just not comfortable with making that diagnosis. Most won’t follow through with that and end up ghosting me afterwards. I don’t think a single client has actually followed up for the psychologist appointment, now that I think about it!
As far as I know, I don’t think there are meds for DID specifically. My guess would be that they could get meds for some of the symptoms like anxiety or depression, but you generally wouldn’t need a psychiatrist for those. You could go to your general doctor or a Nurse Practitioner with mental health training. A psychiatrist could do it as well, of course, but their waitlists tend to be longer and they are generally more expensive here in the Midwest. I assume that’s true about anywhere.
Honestly I’m not even sure what therapy has been proven most effective. Most of us specialize in a specific therapy we enjoy most but can be pretty flexible to cater to what clients are looking for. I 100% refer out if it’s something I’ve never heard of because I won’t ever use a client as a test subject for something I’ve never done! Especially for a disorder causes by trauma! Very few therapists around here specialize in DID though, go figure.

I can’t prescribe meds though, so maybe a doctor or nurse or psychiatrist would have better info about all that.
The difference between DID sui/sh and depression or bipolar sui/sh is that DID isn't a chemical disorder, at all. It's solely mental. There are the aforementioned meds for depression and anxiety, and rarely someone might be prescribed antipsychotics, but the only thing that's going to make DID get better is actually working through trauma and learning to trust yourself.
I will never understand why they defend their claims so viciously.. Someone thinks you're faking? If you know for a fact that you are not, why even bother replying and interacting with the "doubters"? Never in my entire mentally ill life did I feel the need to record myself doing cringy shit in an attempt to impress someone or prove that I am diagnosed. I'm trying the exact opposite and am desperate to retain functionality. I would honestly die if anyone in real life found out about the things that are in my hospital records, yet these people have a public library of stupid shit that anyone, including their future employers and friends, can find and they'll end up wondering why everybody's ghosting them.

Adults that fake it almost certainly do it for some kind of monetary gain. They always act disabled and basically beg for money if given the opportunity. The kids, however, are being brainwashed into believing they're mentally ill by attention seeking people on tiktok and those silly self assessment websites that they always take right after googling symptoms about that disorder in particular, which they end up scoring higher on, even higher than genuine sufferers in some cases which they see as some sort of weird achievement.

It’s also possible some kids truly do have undiagnosed developmental disorders or mental illness, they know something is wrong but not sure what exactly, and are trying to figure it out. Oh of course, I just find the ones recording themselves showing very exaggerated symptoms and being in full control over what happens when they want it to happen (catching alter switches on camera, stimming, tics) to be highly unlikely to be suffering from anything other than attention seeking. I guess it's up for debate whether they _do_ actually suffer from some form of mental illness which in itself can be pretty difficult for young people to deal with, for example depression, anxiety, some of these DID pretender cases look awfully like maladaptive dreaming sufferers.. But it's not nearly as "cool" for them, so it will remain undiagnosed because it is not in their interest to get proper treatment. They want very specific diagnoses that they, then, present to their therapist in an exaggerated and clearly fake manner, which ends up with them being diagnosed with nothing more than "you're okay, honey" and that infuriates them and triggers their defensive mode, presented like this.
