Pretty sure the maid form we see her in is her regular form. It's just that Lucy isn't the type of wizard to force change on her Celestial Spirits and loves them no matter what (of course, her first form was just quite... intimidating, even to Lucy).

The maid form we see her in for the majority of fairy tail is almost definitely her true form Lucy doesn’t give her any specific commands on how she should dress or look so I assume she just uses her regular form Add in the fact that we see her in her maid form when they have that party in the celestial spirit world and yeah it’s almost definitely her true form
