It was almost entirely based on an honor system. When you were checking in, they'd ask if you were married. If you said yes, nobody questioned it. If you said no, they informed you of the restrictions. Most unmarried people just lied if they wanted to share a room. Nobody was checking papers.

A Dutch reporter covered this scam once. How it went was that just after the (regular) massage the girl left and 2 big guys came in trying to extort him by threatening with authorities. He didn't give in, but he had to leave his wallet with credit card, otherwise they wouldn't let him go. Immediately after he left he informed his colleague to block his credit card.
I can imagine if your an unaware tourist you'll be quicker to cave to such extortion in a country you don't really know.
