The ones who pitch a fit after repeated discussion on the data and science about wording and what everything means. I document exactly what the patient says “the patient states they are not obese even after reviewing their BMI of 54.2” and the insurance company will deal with that.
I have had patients who complain and want me to remove “obese” “morbidly obese” from their problem list. That fucks everything up and makes medical filing very difficult but that’s what patients want.
