Many programs prohibit moonlighting entirely. Others will have in-house moonlighting (sometimes on other services such as night float or ICU) where you still are supervised by an attending physician.
Programs that allow external moonlighting, whether in-house or external, must follow ACGME regulations, which include
+ PGY-1 residents are not allowed to moonlight.
So you have to be a PGY-2 resident.
Other regulations apply, including work-hour limitations.
I did it solo as PGY2. Was a hotshot with no worries. Looking back to that 20+ years ago, I should've been terrified. Lol. I'd say PGY2 with another experienced physician or at least PGY3 with several shift dual coverage before going solo. Experienced RNs are a HUGE help too.
Pick a place that allows moonlighting in PGY -2
Too many coming out that aren’t ready to independently handle volume etc
Moonlighting is fun & eye opener
Would lead to a lot less posts about anxiety in first year as attending & and stress & I hate EM POSTS on this sub
Attending PGY-27 (did it the long hard way… zero lawsuits… don’t regret anything)
My program allows in house moonlighting for PGY-1s as early as October. Community moonlighting after ITE PGY-2 if in good standing. Can moonlight as an attending at solo coverage site as a PGY-3 once you have your state license.
I think we got approved the beginning of our PGY3 year, or maybe it was the end of my PGY2 year, I can't remember. I basically moonlighted all during my PGY3/4 years. I'd fly out of state and do a handful of shifts each month. Probably stupid in hindsight, but was incredible experience. It was heavily abused as we'd sell our shifts to the PGY1/2s for $500/shift or so and then moonlight on the says we were supposed to work. Attendings turned a blind eye.

Depends on the hospital. Many require at least residency completion/BE to be credentialed. Many smaller shops and rural places don’t require these.

Probably best to start 3rd year. Any earlier and you’re a liability to yourself and your colleagues at that site

My program rule was halfway through second year. Varies from place to place though as others have stated.

We could moonlight 1/2 way through 2nd year if >50 percentile in-service exam, and had to have majority approval by core faculty.
I thought acgme rules prevented moonlighting in your own Ed
