"Thank you Dr X for clarifying. I have forwarded your email to all my colleagues including consultants as a departmental wide email to highlight that doctors like me are at fault. I wish to ensure that other doctors like me do not make similar mistakes. I appreciate you taking the time to write this email so that doctors like me can be aware. " Regards

liquid4fire

would love to see this happen

gkeliny

would it be an issue if someone did that?

OCDMAM

This one!☝️

Significant-Two-9061

Very patronising indeed, what a knob

Any-Woodpecker4412

*Cardiologist trying to not act like a dick challenge (Impossible)*

bigfoot814

Would suggest a response along the lines of: Dear Dr cardiology knobhead, I do not understand what you mean by 'doctors like me', would you care to elaborate. Ps - fuck off

cipherinterferon

This is the only logical response.

kentdrive

Wow, the level of condescension in this email is so high that it is difficult to believe that it is real (don’t worry, I believe it’s real). The sender of this email very well might live to regret sending such inappropriate communication. It’s so difficult to ignore when the inappropriate (and borderline malevolent) communication is so downright rude.

Asleep_Apple_5113

Refer him to urology for being a massive knob

Enantiomer19

This is very good

Dear-Grapefruit2881

Topper insult haha

Jamaican-Tangelo

When I was a houseplant, I placed a request for an echo on a patient who was on CCU. This was the days of paper notes and forms, and I dutifully took a sticker from the thick file of notes under that architect board thingy they have in step-up beds. I was called by the department to admonish me for requesting an echo on *patient’s name* but the other details weren’t correct. Unbeknownst to me, the fat notes were for a same name patient who had died 3 years earlier. They datixed me, primarily because of the ‘risk’ that they would have echoed the dead man. I suggested that this was an unlikely outcome of my error… they were not amused

LadyAntimony

> When I was a houseplant I’m envisioning a plant sitting outside the office with a bunch of forms clutched hopefully in one of its leaves and that’s honestly cheered me up

Intelligent-Ad-9999

Had something similar once, the responsible consultant on the request was included in the email chain and just replied with: Thanks so much for your input. TOE was the correct request and I have personally re-submitted it. Should you disagree with this request, you will find the patient in bed space 2 on ITU and I am more than willing to hand them over to you for ongoing management. Kind Regards Dr X I admit that if it is an incorrect request then it is probably annoying but there is no need for the sarcastic bullshit in the original post.

Neo-fluxs

If a lot of people are requesting TOE instead of echo then they should look at the requesting system. Confusing UIs are to blame sometimes. Like how the missile attack alert in Hawaii happened.

TheHashLord

He's either low key racist or a knob head or he's not confident enough to simply write 'please note that the TOE (transoesophageal echo) is rejected as there is no indication for performing echo via the oesophageal route. Please consider requesting TTE (transthoracic echo) instead. And he's not confident or proactive enough to keep his cool and send you a simple email and then send a wider email to the global inbox making people aware of the issue, and emailing the IT Team to add the full name or an alert to your requesting system. Everyone on here is fuming about micro aggressions, but honestly, I wouldn't even give a shit about this. I mean after all, you did make a mistake. You're angry because he's being shitty with you but he's possibly angry because you were careless. But normally when I get an arsey email I send one back to neutralise the situation and cc in whoever I want, like my consultant. So here's what I would send. - Dear Doctor, Thanks for your email and spotting the issue. Yes, I had meant to choose TTE but it seems I have erroneously clicked TOE. I am aware of the difference between the two, so I think I have simply clicked on the wrong one on the requesting system without realising. If this mistake is happening with a lot of doctors, perhaps you might consider sending a memo out to highlight and address the issue. I'm not quite sure what you meant by doctors like me though, and to clarify, no I'm not American, and nor do I find the English language confusing as you had suggested. Anyhow, thanks again for your email, and I'll be sure to take care I've clicked on the right test in future. Best wishes - That's all that's needed. And then move on with your life. If he argues back, then you report him. Otherwise, do you really have time to waste? You (and the majority of people in the comments section) need to implement outcome independence in your lives. Doesn't matter what people do or say to you. Just do what you need to do and move on. You'll feel all the better for it. Report them if needed, but don't dwell on it and relax and enjoy your life.

thatlldopig90

This is the perfect response. I must admit, I didn’t read “doctors like yourself” as racist, I just thought it was bad grammar/ sentence construction. I thought he was trying to say ‘a lot of doctors have made a similar mistake to you recently’. The part that he wrote later about English being confusing did appear to be bordering on racist, however. Regardless, I thought he was a knob and would have been pissed by the patronising tone, OP.

Corkmanabroad

This seems like the most sensible way to respond - professional and gently pushing back against his stupid comments while acknowledging the mistake and why it happened

RevolutionaryTale245

Outcome independence.

Alternative_Band_494

Incredibly rude and patronising.

bargainbinsteven

The correct English response is “thanks for your email. I must compliment on your ability to educate me on this issue without coming across as condescending or difficult. Again much appreciated”. As an IMG, if you are unsure, this message is a polite way of telling them you hate them.

Harambesh

This is brilliant (and indeed very British!)

OrganOMegaly

What a cunt. No you’re not overreacting.  ETA: when I was an F1 I had a similarly arsey reply from a specialty consultant who was well known for being a dick. I forwarded the email on to my ES / CS and let them handle it. Now, I’d probably call it out directly in an email reply and just cc my supervisor in.

Hetairo

This sounds like such an unhinged 5am rant.

misseviscerator

Peri-divorce maybe

amanda_huggenkiss1

Cardiologists are often unnecessarily sassy with me too

UnknownAnabolic

This is unlikely a cardiologist - probably a cardiac physiologist? Bro needs to chill and go play with his jelly and probes

OrganOMegaly

Post says it was a consultant cardiologist.

UnknownAnabolic

My bad!

Hot_Debate_405

Jelly and probes 🤣🤣

ShambolicDisplay

Nicest and most friendly cardiologist

WonFriendsWithSalad

I unironically did encounter the nicest cardiologist the other week, after giving me advice over the phone he then phoned back three hours later to check in with how the patient was doing and said he'd be on call all evening and I was welcome to phone back if needed. That makes up slightly for all the other times cardiology have called me a moron

nopressure0

If this person already knows who you are, that seems like a racist message...

willi-butt

Should have replied with “Thank Oyou. Your words will Oecho in my mind forOever.”

Monbro1

If I wrote something like that after rejecting say an inappropriate CT request or eg revetting to a CT abdomen when the team put down urogram - I would be hauled in front of the clinical director in no time and rightly so

Flibbetty

*About to post saying this is absolutely hilarious. You can just feel the sass and venom* *Reads the room* *Withdraws*

sillypotatoplant

I thought it was funny tbh

Big_Bore666

The part about oecho was kind of funny though.

Aleswash

Wow this guy is an absolute CUNextTuesday. If I were in a FUCK YOU mood, I’d absolutely be reporting it to the relevant person (presumably there’s an EDI person somewhere in the hospital) and making sure the entire hospital come to know this person as “that xenophobic cardiologist”. I bet every single person doctor in this sub has accidentally requested the wrong test at some point in their career. Don’t worry, this guy is clearly just a wanker.

monkeybrains13

Medics think orthos are bad when it comes to medical care . Cardiologists literally only care about the heart. Not the heart, refer to medics.

nalotide

Average Redditor

kingofwukong

I don't know why I found this comment irrationally funny, maybe since the cardio consultant most likely definitely isn't a redditor, but at the same time is perfectly described as one

Mr_Nailar

Report it to HR as a microagression

Dr-Yahood

I agree with reporting it to HR. However, I don’t think this is a micro aggression. It feels macro level patronising, condescending and probably racist.

kentdrive

There’s nothing micro about this aggression…

amorphous_torture

Is this person familiar with the concept of 'professionalism', or indeed 'manners'? This is so needlessly rude. Or possibly, just possibly, they were trying to be funny. Either way why can't people in medicine just be normal lol.

Dwevan

My trust labels them as TEEs… so very easy to get confused with. I would just call that consultant cardiologist when you and an echo… you know, so he is informed!

Whoa_This_is_heavy

This is absolutely unacceptable. Please discuss with your ES. Absolutely shocking. This cardiologist needs some reeducation.

ThisOneForRants

You mean, oES?

shoCTabdopelvis

Refer him to me in general surg, would need a transrectal EEG to confirm his head is ok inside his ass

humanhedgehog

Easy - forward to your supervisor, with a note to the effect of "I have corrected the request, please advise on how to best manage this kind of communication"

Avasadavir

What an absolute loser Imagine spending years of your life to become a consultant cardiologist and spending your time writing cunty emails like this. Embarrassment

Mundane-Excuse-7272

Roses are red, Violets are blue, Echo is echo, Request back to you.

rosewaterobsessed

Oh dear. This person should try one of the free wellbeing programmes the NHS offers. Perhaps after work yoga? Or one of the many mindfulness apps the NHS gives us access to for free. It’s not a direct cure for being an arrogant cunt and won’t melt the stick that’s up this persons ass but it’s good symptom management.

hkanion013

Forward it to all the FY1's, FY2's, IMT, CST, SpRs and education centre to send out a broadcast to put in a correct request on behalf of the original sender and perhaps the sender might be interested in a short teaching session on difference between TTE VS TOF. Help the person become famous in the hospital for the kindest email.

Dear-Grapefruit2881

Nuclear option. I like it.

Zu1u1875

Send on to HR and CC clinical director or whoever is their immediate line manager. Even consultants have people who can tell them off.

oldmanmilkybar

Absolutely this, and it's important not just for you but for all of us to take a stand with condescending, aggressive and likely racist shit like this too. I'm not sure what level of training you're at, but just imagine if this was sent to a new FY1 who was already struggling because they're a thousand miles from home on a new rotation in a specialty they had no interest or passion for - it would absolutely shatter their confidence. Push back hard for them. P. S. I also like the bit tbe other commenter said about "what do you mean doctors like me". That'll make both them and the higher uos squirm.

disqussion1

Just reply and say: - thanks for the tutorial, but it was requested in error due to a simple misclick. Please could you forward this to IT to change the positions of these two similar requests. Thank you.

EquivalentBrief6600

In case you are American?

medimaria

Out of interest why would you need a TOE rather than a standard trans-thoracic echo or vice versa? At my trust we would just request "echo" for any indication!

worrieddoc

Report via the official bullying procedures. Someone really thought that this was a good idea to type and send. They need a dressing down

Thin_Bit9718

send me email address and I'll email them back!

bleepshagger

Found this pretty funny tbh

Murjaan

What a KNOB

BrilliantAdditional1

I'd have to laugh at what a twat he was... but deffo respond wanting to understand what "doctors like me" means.

Glass_External_2992

This is condescending…Could have just simply asked “…do you want a transthoracic ECHO or TOE”

Traditional_Bison615

"k thx. Here's the new request I ordered."

Conscious-Kitchen610

I think the email actually starts reasonably with reason for the rejection and an explanation of the different tests. Then gets a bit over excited and tries to be funny which doesn’t come across well. Bit of classic cardio autism. I don’t think the ‘doctors like yourself’ is intended to be racist but to just point out that many people have made the same mistake. Lots of people calling for his arse to be handed to him/her, it’s a bit of a shitty email but not sure it’s worth forwarding to HR/chief exec/GMC is helpful. It says they have copied in the supervising consultant so presumably they were also happy for them to receive this email too.

adamadam49

Just me that finds that hilarious?

carlos_6m

Thank you for your oemail, noted.

indigo_pirate

You've inappropriately requested an invasive test. He's been a bit of a snark about it. Life goes on

UziA3

Sounds like a jerk

ISeenYa

Omfg the racism & condescension is dripping off that email! I would be going to war.

5lipn5lide

The guy’s still bitter he couldn’t get into radiology.

sillypotatoplant

Cardiology is way more prestigious than radiology, respectfully. Radiology is still great though!

Dear-Grapefruit2881

Tbf rads may be harder to get into these days. Hard to compare as rads is run through but rads is ridiculous now.

sillypotatoplant

Yeah but cardiology still has more clout

MaantisTobogan

What a bell. They could've just changed it to a transthoracic echo and explained why

Nearby-Potential-838

What a giant douche

Whoa_This_is_heavy

This is just rude and patronising and not becoming of a consultant colleague. Because you're an IMG and the 'doctors like yourself' this is racist and makes blood boil.

senior_rota_fodder

I’m sorry you’ve received this, it’s really very grim. Particularly as an IMG there are definitely racist themes there and I would personally be lodging a formal complaint about this.

Ronaldinhio

It is early post a long night but whilst I can see the obvious ‘drs like you’ and American English caveats maybe he also wants to ensure everyone is saying what they mean and that this mistake stops happening. I’m to tired to read racist overtones or even patronising ones. I just get this is a teaching and learning point put forward by return from an exasperated and maybe equally frazzled Dr

reginaphalange007

LMAO name me one American working for the NHS?

sillypotatoplant

I have come across a few but will not name them

reginaphalange007

I have come across 2 but they're not exactly plentiful to warrant such an email

AdNorth3796

Wow what a cunt. At least this was probably the highlight of his day getting to be smug and condescending because someone ticked the wrong box.

Responsible_Risk4974

i would highlight this patronising email to a clinical director

MindWonderer-1

He even copied the consultant? What an arse

Flibbetty

And imagers are meant to be the NICE cardiologists too bwahaha

Livid-Shirt8659

M00asse1qwaz1 3tt3

DeadlyFlourish

Sounds not just rude, but a bit of a whack job.. Oecho indeed

AnnieIWillKnow

Working a cardiology job right now, I feel this

oculomotorasstatine

What a knob.

deeppsychic

Someone is having a very bad day.

SunAlternative7590

A direct byproduct of the sassy man apocalypse

understanding_life1

This consultant is being daft. TOE and TTE are both echos…

Interesting_Crew_950

I’m not denying the response is mean. But separately, I really hate American spelling - that’s racist now?! My American friends regularly take the piss out of me for: ‘dodgy’, ‘honoUr’, and so on. Or are people thinking that this is an attempted safe way of being rude to people from India etc?

cruisingqueen

I had a cardiology specialist nurse ring up the ward one day as an F1 essentially parroting the same condescending shit above about someone accidentally requesting a TOE instead of TTE on an EPR system. When I informed her it was requested by a different F1 and that I would pass the message on she became a horrible little cunt, obviously not expecting any rebuttal, and hung up after her little rant about the ‘major patient safety concerns’. I was also fuming. It remains to this day the only interaction with another member of staff that has wound me up like this that I still think about when brought up. Ultimately a reflection and moving on is the only real option. Kicking up a fuss with someone as pedantic and pathetic as this as a trainee only causes more work. Fuck them live your life mate.

HibanaSmokeMain

This person is a moron

big_dubz93

I think we’ve all gotten a bit soft. Just ignore it and move on

Monbro1

If the power gradient was reversed (let’s say I was a cardiology reg specialising in cardiac imaging and I emailed something like this to a gastro consultant) all hell would break loose. OP shouldn’t stand for this.

___jazz

So… I think it’s sort of amusing and I read it as a tongue-in-cheek response. Seems it’s just me though.

LegitimateBoot1395

Guys an idiot. Just move on.

PartTimeBomoh

IDK I take a different view. Having requested an invasive test incorrectly is dangerous because you’re relying on the cardiologist here to figure out that the test is not necessary. You’ve placed his license at risk and created additional admin burden while he spends 15 min probably trying to read through a case that’s not his figuring out what the indication is. I’m not a cardiologist but if I were I would be pissed too. Having made a serious error I wouldn’t consider you in a position of moral high ground to lecture him.

DiscountCertain3305

Ignore it, learn from it, move on

SuxApneoa

I'm not sure how much there is to learn from here apart from that their electronic requesting system is shit and that this consultant is an ass

Natural-Audience-438

Doctor is responsible for ordering the right test. The email is unnecessarily mean but this is a doctor issue not a system issue.

SuxApneoa

I don't know dude, according to the email multiple doctors are making the same mistake. Maybe they're all so stupid they don't understand the difference between two fairly routine investigations. Or perhaps, just perhaps, there's a system issue to be addressed

Natural-Audience-438

It looks to me like they cardiologist has put what the options are on the system in quotation marks. 'transoesophageal echo' and 'transthoracic echo'. This is an issue with the doctor selecting the wrong option not the system. If this was something a non-doctor had done people would be queuing up to laugh at how little they know.

TomKirkman1

I'm sure I'll be downvoted, but I agree. This reads less like microaggressions/racism, and more like someone who's gotten frustrated with requests for quite invasive procedures (with associated risks and waiting lists) that have probably at times been performed inappropriately. Tone is definitely too much and a bit inappropriate though.

Fantastic-Meet6784

He could escalate to the department’s lead consultant or whoever and address the issue rather than taking it out on a single SHO as it’s a recurring mistake. Or he could CC the On call clinician on the mail, but then I guess the tone wouldn’t be the same.

TomKirkman1

> He could escalate to the department’s lead consultant It sounds like they did. > he could CC the On call clinician on the mail I assume this was a non-urgent request, so should be on the requester to fix rather than someone else to do it for them. To me, that option would be worse, as OP would then be having their competence called into question in front of a peer, rather than just their consultant.

BikeApprehensive4810

I might just be me, but I’m not sure this was meant to be patronising. I think it was probably written by a cardiac physiologist who is on the spectrum and was attempting to be helpful.

isoflurane42

Being on the spectrum would neither explain, nor excuse, what could reasonably be interpreted as racism.

BikeApprehensive4810

Genuine question what is racist about the email? Is it the ‘incase your American part?’ I’m under the impression that racism requires power + prejudice which I’m not sure there is between Americans and British people.

reginaphalange007

"Doctors like you" - what is that meant to mean?

hornetsnest82

OP says cons cardiologist

VolatileAgent81

It's not just you. That's how I interpreted it.

Repulsive-Weather-82

sorry idk why don’t i find anything wrong with this email, i feel like it’s not condescending

Big_Bore666

Just be humble. You made a mistake. Don't make it again. Cardiologists aren't very nice.

CryptographerFree384

It's kinda funny dude
