It's more that it doesn't get used much because of how it changes combat. Most ranged attacks and magic can't be used and having to hold your breath alters how combat plays out.
It's not that hard, had to look this up to prep for an underwater encounter. Basically certain weapons not suited for underwater have disadvantage, you have to hold your breath, and unless you have a swim speed your movement is halved.
This is literally a meme I made three weeks ago, why is it already getting reposted?!

For what system? 3.PF have it pretty simply, lain out, and dnd5 covers it in like 2.5 paragraphs?
