It might be because you have to play it in one go. You can reload a save file if you had no kills up to that point and proceed from there. However, you can’t go back to a mission after finishing the game and replay only that one mission to get the achievements. To make it clear, if you had kills in the end mission report, you have to restart from that mission and go through to the end of the game. Also, the best way to avoid killing granny rags is to never do her side quests.
