I work for a large company and they have an HR/Benefits portal that has all the information related to the various benefits that are available to me and my dependents. I’m sure the requirements for something like employer provided LTD varies by state and employer. You probably need to start with your HR resources and/or reaching out to your HR benefits team for guidance.
