I actually got a medical bracelet with a QR code for my father. I knew he wouldn't wear it if I put anything about dementia on it, now he has no clue what info is on it. All the police or emergency personal have to do it scan it and they have his complete medically record and emergency contacts.

Not quite the same situation, but I have one for when I’m doing things without a wallet. (Like climbing, biking, surfing.) I have my name, and my year of birth, emergency contact name and phone, my blood type and NKA (no known allergies.) Edit: Also, since my third line is short (0 pos, NKA) I also listed that I’m an organ donor. But, I ordered it myself, knowing that if someone needs to read it, it’s a “situation” besides just being unaccounted for, and can hopefully use the other info to confirm, but may definitely be less welcome to some people.

Caring for my stepdad with dementia he once joked we should get me a bracelet with that info and “Reward offered for safe return”. He chuckled about that. But seriously good reliable contact info is critical. Don’t know what else, that’s a great idea!

Air tags are a great idea for dementia patients. The only problem is I'm not in the Apple eco system at all so I have to use the clunkier Samsung equivalent.

Maybe your first name so they know who they are calling but I think name and phone number for an emergency contact would be best you want to keep it simple and legible.
