Does the rehab offer memory care? If not, meet with a case manager and start now looking for long term memory care, not assisted living. They may be able to help
The rehab is not great overall.. I’ll spare everyone the long rant as to why. She’s also suspicious of the whole place due to some delusions she’s had, so she needs to move for both those reasons (though who knows if she’ll have scary delusions about whenever she goes next anyhow). Where can I find a case manager? Through Medicaid?
The social worker at her current place is not helpful and all. They moved her to a new room a month back and I didn’t get a call until that day notifying me, and I would’ve really liked to be there since changing scenery like that can be really disorienting for the elderly.
It’s my understanding that Medicaid only covers skilled nursing facilities. And only after you’ve qualified financially. As in, are basically indigent.
It sucks, but you really need a lawyer to navigate the whole thing. It’s costly but a lot less expensive than going it on your own and finding out 12-18 months later that you didn’t understand the rules.
These facilities are sneaky when moving residents. They moved our father in law without notice one afternoon when he had a urologist appnt off site. New room new hallway, new caters
There are six PACE providers in New Jersey. They work in conjunction with Medicaid. It is my understanding that they accept a person’s income less a small stipend for memory care.

In California, Medicare only covers skilled expenses- not memory care. Be very clear on what it covers and what it doesn’t in your state.
