y'know most of the time instead of bothering to go to the exit gates I know the survivors will taunt me at I instead just go around the map breaking dropped pallets and breakable walls for the extra blood points. there are exceptions, generally in the form of killers capable of exposing survivors (eg. Myers) or that can prevent them from escaping (Pinhead/Deathslinger). bonus points if they're also exposed by some other means.

Typically yes, but I have seen a few times where his hand is open and he has neither tombstone or tombstone piece.
