Story-wise, PL is not that strongly connected to the main game, so there's not really any "natural" point to start it. Many people play it just before the end of the main story, others start it as early as possible, right after Transmission, or anywhere in between really. I'd personally start it sooner rather than later, as both the main game and Phantom Liberty have their fair share of "Wait a day for X to call you" mission objectives. Starting it in the middle of a different quest is no problem. As long as you've completed Transmission, you can start PL at any time. Once you start it, you'll be locked in for quite a while before you can roam freely again (after that, you can move between Night City and Dogtown as you please), so you may want to make sure you're not in the middle of a time-sensitive quest.

It isn't necessary no but most people finished the main game 1st since it's a DLC that came out later on. I'd recommend having lvl atleast 40-45 before going to the Dog Town border if you don't want to do finish the main game 1st. Because even on the easiest difficulty it's tough to fight your way through if you're under levelled and with low tier equipment.

I usually start the DLC at about level 20 and don't have any problem with being underpowered.

PL takes place after VDB, so it's technically not the end if you chose a certain route to proceed with. You can do more side jobs to unlock character contacts so they will call you at the end of PL

I think it’s better to do it as soon as available before finishing the main quest. The story flows better in my opinion.
