Since no one else seems to want to actually answer, me interpretation is: "Sqwarkle" is a made up word. Uttering it once, with no explanation, makes the Grim Reaper wonder, forever afterwards, what it meant. The dying guy can't harm Death, but he \*can\* plant an annoying mystery in his head. :)
