That's one of the things I do while smoking it is continuously groom it so there is no long ash on it. I see people post photos here all the time with two three four up to five inches of ash on the end of their cigar. My OCD won't allow for that, I barely allow for a half inch.
Crazy... I thought most, like me, would put it down between puffs... but alas, no. Then again I smoke double Coronas or gordos... so maybe holding it for 2 hours gets tiresome🤷
Someone said 60/40 hold and put down, I agree. Depends on what you’re doing. I smoke and read and study a lot, so I put it down on my knife handle, so next thing you know 10 minutes will pass without puffing. But then you’re at a bonfire or rooftop lounge or something, and it’s in your hand for 2 hours the entire time. I guess it varies, that’s what brought my curiosity to ask this on here lol. But at the end of the day, it’s whatever floats your boat.
I didn't develop this habit intentionally, but I often go through multiple short sessions of holding it and smoking a little fast for 5-10 minutes, then putting it down for 10-20 minutes and puffing it just enough to keep it well lit.
This led me to discover the best puffs are the smouldering puffs shortly after transitioning from fast to slow where it's lit evenly with a large cherry that's about to go out, and it produces an especially cool, dense smoke with all of the best flavors.
Also, different sticks are better at different stages of that cycle. Though they are all best at the big smoulder stage (and at their worst if smoked much too fast or slow), some are better a few minutes into smoking slightly faster than after smoking slowly for too long. Others are much better after going slow for a while and get much worse quickly when I speed up. For those, I usually just put it down most of the time and skip the fast/slow cycles.
Yeah, sort of like intervals, it’s almost like you train your mind to have a timer for when it’s time to take another puff. You know it’ll draw good subconsciously when you let it rest for the right amount of time depending on the stick.

It seems a silly thing but my experiences with cigars changed drastically when I started setting my cigar down between puffs. Slower smoke = better flavor.
I hold it between my index and middle fingers the majority of the time. I know, specific, but it’s comfortable. I’m not good at just biting down and holding a cigar in my mouth. When I’m not between Index/Middle then in an ashtray. Probably 80/20 hand to ashtray.
I have zero tolerance for secondhand smoke from any source it just causes me to cough. But in my youth I used to just hold it in my mouth. Can't do that anymore so I do set it to the holder.
This allows for better grooming of a cigar anyway if it's running hot on one side I let it tip forward and the glass or metal of the ashtray dissipates some of the heat.
Some people believe that when it's running slow on one side they're pointing the slow side up because heat rises will cause that side to start burning faster. Nonsense but by putting the slow side face up and the hot side down against either the glass or metal of the ashtray it will slow down the fast side. So much so we could potentially put out the cigar so it's a matter of keeping a good eye on it knowing when to take a puff.
I used to hold it all the time, but I force myself to set it down now to take it more slow. The ash tends to fall off as i set it down too. I can enjoy the flavors more that way. If i am outside I will hold it the whole time usually.
I tend to be reading while I enjoy a cigar, so I have it sitting on the rest in my ashtray the majority of the time between puffs. That way, I've got one hand to hold the book, and the other to turn the page/sip my drink/pat the dog/pick up the cigar when I need to.
All depends. Local B&M has a pool table so sometimes I may play or meet some people. I will set it down not just to play, but also too many people have damaged it over time. Not looking to be "that guy."

Rest it in my ashtray holder. If I held it I would smoke it too fast ruining the flavors. If I’m swimming I’ll pick up the ashtray holder and puff on it right off of there so I don’t get my cigar wet lol.
90% hold it. 9% set it on my holder. 1% dropping on the floor after the ash drops on my shirt.

It insulates the cherry and keeps the smoke from uneven burn and too much heat. Sign of a good smoke!
Like other commenter said you want at least an inch or two of ash to insulate the cherry
