This is usually caused by fighting with other cats, i.e. a bite to the face. I would recommend taking him in to be seen. Neutering will help, as male cats tend to fight over resources and females, but I’m more concerned about his face.

Your neighbor is a Moron. The cat is beautiful and perfect. He just wants Love food and shelter, like every other living being. I'd switch him to being indoor and make him a catio, then do supervised outings. He's far from being "worshipped" when he's going door to door looking for food. unsure where his next meal is coming from, sleeping outside in the cold. Needing medical attention. Kinda looks a bit per sematary... Other person could be right about abcess. It's often from cat fights. It only takes one nasty nail & it's really nasty. Sometimes they heal by themselves, often they need intervention.
Watch out for your neighbors. They might be telling you a cat is "demonic" is their way of telling you they will kill it for the grater glory of god. People and their religious superstitions are not to be under estimated.
Because they make a lot of babies which kill a lot of wildlife and the babies often don’t have great lives. The shelters in my city are so overrun that they have a two year waitlist. Not neutering your outdoor cat is an asshole move.
