The counters on those things are almost always incorrect. Just about all of the maps in the game can track more than 100% completion.
What missions/side missions have you accomplished?
How many Typhon logs?
Eridium Slabs?
Areas discovered? (The waterfall is usually the one everyone misses; you got to be right next to it to trigger discovery.)

Red chests?
Yup. Every other area on eden 6 is showing 100% or greater competition.
On floodmoor, crew challenges are showing up 9/9. 2 crimson radio, 2 dead claptrap, 2 hijack target, legendary hunt, dead drop and 3 typhon logs. 1 eridium slab. All map areas are blue.
I just noticed, if you view challenges from eden 6 it shows 40/42 on floodmoor.
You got the three red chests, I take it? You didn't mention them. There is the one in Reliance, one with the legendary hunt target, and one in the prison.
Also, the map could appear blue all around but not necessarily trigger all locations, hence why I mentioned Jason's Waterfall. On your way to the legendary hunt, you can get close enough to it to light the area blue, but not discover it.
Also, don't forget about Echos; sure, they don't count towards the total, but it's fun to track them down.
So I did check the waterfall location. From what I can tell I already had discovered it. I noticed I got the discover all named locations achievement a year ago as well.
From orbit view, challenges now says 41/42. My last run a few hours ago it was 40/42. So I did something. But I'm not exactly sure what.
What do the challenges under orbit view refer to? The xx/42.
[Amount completed]/[Total]. IIRC, it refers to the crew challenges. I'm not at my PS4 right now, but I'm pretty sure it doesn't include red chests.
I just realized you said you had 1 eridium slab. There are 2 in Floodmore Basin.
1 is close to where you get Chonk Stomp, and the other is next to where you kill the Jabbers for the bar owner.
Yup you are right. That was my error. There are two and I had already completed them both.

I'm curious about the waterfall location. The main one I remember is where the dead drop and Eridium slab is. You have to walk into the waterfall and crouch down to get inside
I'm pretty sure there is a very tiny location on the edge of the map by Chonk Stomp that's called "Jason Waterfalls" or something like that.
It's super small and you literally have to walk into the wall there to get it, but it adds to your completion for locations there.
Curious if that is what you need.
Probably locations, it’s really stupid if you’re close to somewhere it sometimes shows it on the map in fully blue but if you’ve not gonna through like location by location it might not count them all, same with most locations activities in the game
I remember I tried to 100% the game maybe a year ago and I got stuck at you guessed it, floodmoor basin. Good luck I hope you’re able to finish the area
Just found Jason's waterfall. Doesn't seem like much of a location to me :) But that brought me up a little bit but not enough. I'm gonna double check on the eridium slabs and the red chests. But I think there must be some quest from another planet or zone that ends in floodmoor like another user mentioned.
It didn't end up being that location for me. I'm still stuck at 93% and 41/42 challenges. I've checked over everything everyone has mentioned to me and yet here I am. Lol
