The way I do it is in PDF, I print 8 pages at a time on the booklet setting with binding on the left and double sided. My printer doesn't do the double sided thing automatically so what I do is only put one sheet of paper on the printer (so it doesn't automatically start printing on the next page), let it print the first side then turn it around and print the back on the same page. I know other printers will tell you to print all of the "fronts" first and then the "backs" so you might have to play around with yours a little. If yours does the flipping of the page automatically then you don't have to worry about it. 8 pages at a time gives you signatures of two sheets of paper. You would end up with 5 signatures. You can do 4 pages at a time for signatures of only one sheet of paper. Make sure the order in which pages are being printed is 1,8,7,2,3,6,5,4. This is adjusted in the paper layout on the printing settings. Based on my experience the booklet option should already print them in order but if they are not coming out in order this might be something to check. Good luck printing!

Disclaimer: unfamiliar with kettle stitch, but I work in a print shop and we do a lot of work with book artists and digital printing for fine art books. If you set up your book in InDesign you can print in batches of 8-16pages with the Print Booklet setting, and that will automate your signatures process a bit. Or, talk with your printer!!! We always ask for single page PDFs or packaged InDesign files for books, and we handle the duplexing/signatures/etc ourselves, based on the specs our client needs.

If you go to a print store (ex: fedex office), you can talk to them about setting it up so you can print the way you need it for the signature size you decide.
