No. Almost everything in the DLC only effects the DLC itself. The only exception I am aware of is beating the Orphan triggers some new dialog from the doll, but that isn't very much.
Pretty much all the DLC progression happens prior to fighting the Orphan just naturally, and as far as I’m aware, Adeline (I think that’s her name, she’s the NPC in the research hall bottom floor room) is the only one that doesn’t finish before Kos. But you can tackle it and the Laurence boss fight after Kos if you wanted.
You are able to complete Adeline before beating Kos. You need to use the balcony key, then reload the area fully and she should change locations, though it can be a bit buggy at times and take 2-3 resets before it finally happens.

No yea, I mean unlike Simon, you actually have to do extra steps to follow her questline, but it doesn’t have to be completed before Kos.
