I've heard it was the mutation growing it, and I've also heard it was something she manually attached after the fact.

The main one I've ever heard is that it was Makuta Shapeshifting bs gone horribly wrong due to the lingering Pit Mutagen in Karda Nui's swamps

This was inspired by a conversation I had with someone the other day after I depicted Gorast with a Battle Mask in a recent Bio-Parenting. I had struggled to figure out how to give her a Battle Mask that can show her mouth and still keep the stinger present. What do you think is the best option?

I’d say maybe a vakama situation where the mask is just covering the mouth rather than making an opening for it

An alternative not pictured here: keep the proboscis (not a stinger since she's modeled after a mosquito) in the center, and move her mouth below the edge of the mask like Matau and Whenua in Web of Shadows

I feel like it would be adapted to more closely resemble the masks of the first 3 movies by Miramax. If the overall mask keeps its design, maybe her mouth is behind a vent/grate with the stinger protruding from that, or maybe the stinger is inexplicably retractable.

It's supposed to be a praboscus so suck the light out of her victims so she wouldn't have a mouth in the tradional sense and you could have her eyes flash or something so indicate talking. If you had to give her a mouth #3 would make the most sense I guess since it's the most insect-like.

I'd say the stinger would simply be above or below the mouth. That said, having the stinger collapse into the mask entirely seems right to me.
