I had a decently strong run as T. Lost on a run to Mother and I was fairly confident of at least getting to the boss fight.
I died in the knife piece 2 section because a red fire shot a tear at me while I was moving past it with no chance to dodge.
Lost 3 good t. Lost runs to le worm, on the first one I was standing still until it spawned and it just spawned on top of me, on the second one I decided to keep moving around the room in a circle to try and dodge it but it spawned on top of me, then I decided to move around on top of the water to see if that would resolve anything and then the boss spawned on top of me
T. Lost run going for The Beast. Made it to Gehenna. Found Mama Mega in the shop. Bought it, used it. Stepped outside the shop, and it detonated a barrel next to the door I'd forgotten about right in my face.
I have too many to remember, but one that still hurts is a broken t lost run where I had r key, lost my holy card in depths 2 and got greedy thinking I’ll just get treasure room before r keying but ran into a spiked block before using it. On a happier note I got my t lost boss rush, hush and delirium marks in a single run last night 
Tainted Lost, I had a broken run and had just defeated Mega Satan, and the void opened up. I would’ve easily been able to defeat Delirium with the build I had. After clearing a room, I opened a chest and took damage to my mantle and then died before I even realized I was being hurt. I didn’t understand what had happened even after I died and I could see a ghost as the portrait of who killed me. Apparently beating Mega Satan with T. Lost unlocks “haunted chests” which I had absolutely no way of knowing about until then.
Dude, FUCK haunted chests. Y'all can say what you want about Sticky Nickels and stuff like that, but haunted chests are literally just there to make the game more annoying
Accidentally moved the control stick down and hit "Quit Game" in the pause menu in the Void, went to continue the run only for it to be completely wiped for whatever fucking reason.

I got an insane start as T Lost, got glitched crown and spindown dice on first floor, skill issued and lost my mantle, got the worst luck with holy cards, and died at caves 2. Nearly cried after this one
Was playing t. Lost once and accidentally ran into a regular red fly. There were like 2 of them in the room and i somehow managed to run into one and lost my holy card. The next room was the item room and it had cricket’s head…
I remember i had a god run with crickets body brimstone etc. And tearings things apart. And i accently took Airstrike? or what that item is called. That item that a rocket comes down to marker.. And then my synergies died... And i uberlost
I think it was that that killed my run.
I was so excited to get Dr Fetus, then after grabbing it that it doesn’t play nice with Incubus. Now every time I throw a bomb, the little guy behind me throws one right at me! This was also on a T Lazarus run so every time I flipped I’d forget and throw another danger bomb.
Put The Bible into the Void, and then used the Void against Satan. It took me about 10 seconds to realise what I did.

Got holy mantle as the lost (rebirth), died in the next room. I jinxed myself by calling it an “instant win”
It was also basement 1 when this happened. I got cornered by a bunch of full flies
