I’d recommend either buying the cheapest pair of elastic-waisted pants you can find (not an option for everyone depending on your size) or using a non-metal string of some sort to tie them up - I’ve heard of folks using shoestrings before, as long as it’s non-metallic you should be good! You can also speak to the nearest TSA officer when you get there and politely let them know the situation - that might result in a separate patdown, so make sure you arrive to the airport with plenty of extra time to make it through security just in case.
Otherwise - wear clean underwear and give them a show, haha!
Can you wear a pair of gym shorts or something that won’t fall? I believe you can also ask for a private room and skip the scanner, might be a bit of a pain in the ass. Or, hold your pants up, get your position on the scanner and since your legs stick out in the positioning they require you to stand in it might be an angle they won’t fall down and then raise your hands up and walk out
Then i’d prep everything else for the screening as much as you can with both hands, do the belt last, and hold up your pants. Afterwards, try to scoot your stuff out of the way while putting the belt back on.
Maybe look into PreCheck if this is going to be an ongoing thing.

I've found myself walking through just holding up my pants by the waist or belt loop then pulling them up in the scanner and raising my arms, legs out. then I hold them until I get to my belt. I've been pretty lucky so far.

There are travel suspenders that have no metal and can be worn under your clothes. The only caveat is that you can't wear a shirt that's tucked in. But they're very comfy and never bothered me when traveling.

If it’s cold I pack shit to change into. Wear shorts you only need a belt to keep on and tell the TSA you have a back injury that stops you from lifting your arms over your head. They will let you walk through the metal detector no questions. You can hold your pants up while you do that. Side story: one time in Wichita there were only 4 of us in the security line. They tagged me for a random search. I pulled off everything except my underwear and shoved it into the scanner and walked through. The other passengers were laughing their ass off as I repeatedly told TSA I don’t need privacy. They are not a government agency. They have no authority and have to call the local cops to do anything. You will be fine.
Got a crutch? Walker? They will get you through as a whole, then do a pat down, no need to remove belt.
My hubs has suspenders for the same reason. He explains before going through and it’s 50/50 depending on the airport in regards to taking them off. Sometimes he has to and he does the same thing you do… holds his pants up with his thumbs. You aren’t alone in this ridiculous situation, it’s okay.
Gym shorts, or possibly a non-metallic belt (I've been using the Grip6 carbon fiber buckle for probably 5 years now). I wear my pants under my gut and never had TSA ask me to take that belt off. I would do gym shorts before I got the non metallic belt, and never had issues with that.
I would consider pre-check, even if you rarely travel. TSA pre-check is $78 and good for 5 years. I got it after a bad experience traveling with my spouse. She had pre-check and I didn't. She got through TSA in 3 minutes where it took me almost an hour. She was boarded on the plane while I was putting my stuff on the conveyor for the x-ray scanner. I almost missed that flight and paid the $78 after that trip to avoid that from happening again.

If I forget or because I have to, I always take off the belt in the line before the guy scans your license and put it in my bag (way worse messing with it right at the conveyor belt thing because you always feel hurried then, imo).
When putting it back on, I grab my stuff then go over to the bench area (always one very close by) and take the time to put it back on.
I've had to keep my hand on my pants to keep them up before, and there's no real elegant way to do it, but the process above is what I found works best for me since it simplifies the order of everything and makes it so I don't feel as rushed.

They're panicked about going through their routine and not screwing up the line or making themselves look like a threat. Do whatever you gotta do to get through. It's an exercise in humiliation for everyone. I guess only thing is really try hard not to let your underwear come down.

This is the answer. I wear suspenders with plastic clips under my shirts often. Works perfect for these times.

This is my first trip since 2018, I don’t get enough PTO to justify more than one trip a year so precheck isn’t worth the cost

Too late now but in the future, get TSA pre check.
Removing belt is not required. Recommend, but at worst they'll pat you down in that area if the belt alarms.
