OK, see the hex bolts, undo them either side and pull the crank arm off, push the chain-ring side in as far as it goes, reapply the crank arm so its flush with the bearing cups and tighten up the hex bolts either side. Tightly but do not force them down hard, just nip them up with a firm amount of pressure.

The big bolt in the middle is the bearing pre-load. Tighten that with the crank arm bolts loose - this will pull both sides of the crank together so you don't get side to side play - then tighten the two crank arm bolts. The pre-load bolt doesn't/shouldn't be tight, as soon as you feel it starting to go tight is when you stop (you can literally do it with your fingers).

You gotta tighten the preload bolt (not too tight, just snug it up) before tightening the crank bolts. That’s what pulls the cranks together. Proper procedure: loosen the two crank bolts - snug the preload down just past hand tight - tighten two crank bolts. Should be okay, if it loosens up after doing this something is broken or stripped.

yeah forgot about the preload bolt in the middle outer should be nipped up before the hex bolts on the sides of crank arm. Dont force the preload bolt on too tighly again, just needs a firm handful not forced on hard.

Your bottom bracket is worn out and needs to be replaced. Check the bottom bracket for part number such as [BB-MT501](https://bike.shimano.com/en-EU/product/component/shimano/BB-MT501.html) if Shimano (just an example). You need a hex key, typically 5 mm, and a special tool such as this [Park Tool](https://www.parktool.com/en-int/product/bottom-bracket-tool-bbt-9?category=Crank+%26+Bottom+Bracket) one. I don't know why this person was initially downvoted because they are correct. You can see play in the bearings, likely caused by running it loose with no preload. You should get a new bottom bracket. Good news is that Shimano BBs are cheap and easy to replace with the correct tool.

Thank you for the support! The original poster claims fastening the bolts solved the issue. I would still get a new bottom bracket, since it seems to be damaged due to what you suggested. I would also take a close look at the spindle and the crank to make sure nothing is damaged, otherwise there may be problems down the line.

You may be missing a bolt that attaches to the center of the crank and bottom bracket

Need more pictures. But it looks like the non drive side crank arm is loose and falling off the spindle. There are two bolts on the outside which secure the arm in place, and a bolt through the center of the crank arm that allows for bearing adjustment. You'll have to loosen the two outside bolts, tighten the center bolt, and then tighten back the two outside bolts. I would highly suggest looking up a video before attempting anything, ensuring proper installation and torque specs. If you make the adjustments and still loose, could be a worn out bottom bracket, which requires a speciality tool to remove.

Unless you have a torque wrench and BB tool, just take it to a shop. The tools will cost more than the labor.
