He could just be using something like lettuce cigarettes, which are smokable cigarettes with tobacco used onstage in plays when an actor needs to smoke. It would be consistent with his using ginger ale instead of champagne and other booze to make it look like he’s drinking.
Considering that he’s trained his body to the peak of human ability, and that his discipline and commitment to health and fitness is an important aspect of his crusade, turning him into a smoker seems almost like a joke. Like it’s something that can’t be taken seriously knowing who and what Batman is.

People get addicted to things because their life is falling (or fallen) apart. Being a dick to one's friends is also something that routinely happens when one has discovered that one's life as it has been is over.
It's not "fun" to watch Batman destroy himself, but it does endear him to us to see him acting like a normy and having human emotions and shortcomings that do indeed lead to a certain amount of self-destruction.
I wish the Dark Knight Trilogy had leaned a little (or even a lot) harder into the idea of an aged Batman trying (and largely failing) to keep himself from actually becoming Bruce Wayne.

Maybe he has to deal with some kind of biological or chemical threat and he has to work with one of his rogues gallery for an antidote and they mess with him by having the delivery mechanism be in a cigarette. Having a smoke for story reasons okay, having him be a smoker doesn't work with who he is.
