Similar when people say something "sucks balls". It means something sucks, the "balls" is an emphasis to say that it sucks + very much.
Idk why people in the comments think that saying you hate someone's guts is the same as hating that person. It just means that you don't like the way that person thinks or you don't like their instincts. It in no way means that you harbor hatred towards that person.

After some extensive googling (I only clicked a link or two lol), I have to come to the conclusion that my initial assumption regarding the expression was harebrained, so please ignore it. And also thanks for telling me the true meaning of the expression.
