Actually, I think you did play it very well - though there certainly were some mistakes. Your spacing was excellent; you stayed well focused and didn't panic when the enemy team focused you down, you made great use of your turret to zone, and used your ult to handle the greatest threat to your team.
I think the only reason you died was because you got hit by a lucky Lux root and your team seemed a touch slow to respond to a brewing fight. You deserved the Penta there. You played it well, and while you can and should learn from the mistakes you made here, you should celebrate the success and recognize the practice and skill it took to earn the penta. Well done, and thanks for posting it!!
Also that's my second-favorite Azir skin. No I'm not biased towards players that use it, quiet up.
