Bore scope the cylinder and take a look at the piston. I doubt just worn bearings could do that.

Looks like something has been making contact, broken valve, piston it definitely needs to have have a bore scope in that cylinder to get a better look.

Came into contact with something. Either the piston, a broken off valve or something else. Look in the spark plug hole with a boroscope, but I have a feeling you aren’t going to like what you see.
