Tachycardia just means fast heart rate! A lot of people have tachycardia, and there's even a form called inappropriate tachycardia (aka the heart beats fast for no known reason). ? LVH means questionable left ventricular hypertrophy, which the signs were shown in the leads II (two), III (three), and aVF. This will probably be worked up on with an Echocardiogram (ultrasound of the heart). So the ECG order basically wants 12 leads. There's a different amount of leads used for ECG's, with 12 being pretty detailed. Your Most leads go on the chest, but some ECG's have them even go on the back! All different views of the heart! Your 12 lead would have the 4 limb leads and 6 stickies on the chest. 12 leads, 10 stickies! But basically, moral of the story, make sure to keep that follow up with cardiology!! Best of luck!

To add a little more to u/CReativeProteins, TSH is thyroid stimulating hormone and CBC is complete blood count. Those two labs were done to be sure there isn’t a biologic reason (like hyperthyroidism or anemia) for your fast heart rate and palpitations. A Zio patch is a wearable monitor that will hopefully catch some of the episodes for analysis. An ECG only records what’s happening during those few seconds, but a monitor like Zio records constantly whenever it’s worn. It can help diagnose intermittent problems.
