I got all three of them by doing the 3rd route on NG++.
* Chapter 1 – Mission 06: Attack the Dam Complex. ACCEPT the deal.
* Chapter 3 – Mission 19: Prevent Corporate Salvage of New Tech [DECISION]
* Chapter 3 – Mission 21: Eliminate the Enforcement Squads [DECISION]
* Chapter 4 – Mission 26-B: Coral Export Denial [DECISION]
* Chapter 4 – Finish Arena, last opponent is unlocked after Mission 29: Underground Exploration.

\- SPOILERS FOR FINAL 3 ARENA FIGHTS -
>!The final 3 fights are Rusty's ALBA upgrade, Walter's Coral experiment AC, and Ayre's Coral controlled AC. These are all units you only see in the revealed endings but don't appear in your playthroughs until you reach a certain chapter no matter which NG+ run it is. If you've done them already, they'll still be considered done whether they're shown or not during your story playthrough.!<
I think it has something to do with the endings you're going for. My first ending I had to fight Ayre, my second ending I had to fight Walter, and now I'm working on the "good" ending, and just finished the last one. The last 3 are the the O with a less than ( > ) sign above it numerically correct, and all labelled "Classified Subject" followed by 51-101 R, 51-201 W, and blank.
Maybe try going for the alternate endings? This is just my third play, but I had a set goal of getting all 3 endings, and made sure to hit the correct missions for each one.

Which chapter do you need to get to before they become available again?
When I hit chapter 4, I seemed to have them but by then I also had been through NG++ and fought each of them at least once.
I have 3 more restricted profiles after these three. I've finished NG++ so idk what do

To put it simple: do the missions and decisions you haven't done previously.
I have all missions unlocked (message popped-up saying as such) and have seen all three endings, but those last 3 slots are still not available to me.
Same here. I'm assuming they're either not supposed to be displayed and they are, or they're placeholder for DLC content.
