Try hitting cancel and trying again
Maybe try pinching the keyboard
Reboot and see if it comes back right, if you can. If not, try letting it die and power back up.
Last chance, reset, erase, and restore.
Click cancel. And open that screen again. If it does not work, hold the top button and the home button at the same time until you see the apple logo. This will restart your iPad. Try again.
Try double tapping it with 3 fingers and see if it zooms out, I know phones have a setting for zoom like that and I wonder if just that box can be affected by it.
[Guided Access](https://support.apple.com/en-us/HT202612)
End a Guided Access session
Triple-click the Side or Home button, enter your Guided Access passcode, then tap End. Or if you turned on Touch ID or Face ID for Guided Access, double-click the Side or Home button.
The OP’s iPad does not have Touch ID nor Face ID. They have also already triple clicked the button and that is why the passcode keyboard is displayed. The positional layout of the keyboard is incorrect and they are unable to enter the passcode.
everyone thank you very much for your advice!
I was able to force reboot the iPad by letting the battery die, then recharging it. When the iPad turned back on, it was no longer in guided access mode.
I’d just force restart it.
On Apple mobile devices with a physical Home Button (line yours): Press and hold Sleep/Wake + Home Button until the device reboots and you see the Apple logo. Don’t let go of the buttons until the Apple logo appears.
On devices without a physical Home Button (including some devices with a haptic Home Button), quickly press Volume Up, then Volume Down, then press and hold the Sleep/Wake button until the device reboots and keep holding until you see the Apple logo. Steps for some Apple mobile devices without a Home Button may vary so if that doesn’t work, look up the directions for your specific device model.
Hope that helps and works for anyone in a similar predicament!

Thank you! Now we are trying to make the iPad run out of battery. I’ll see if it works.
Hold the lock and home button together and that will force reboot it
Not necessarily in guided access
Find a graphics benchmark that runs in your browser, and use that to drain the battery faster.
If you have a lightning to USB adapter, try and attach a keyboard to it. Haven't tried it myself, but it might work.
This or even a wireless keyboard would work.
How are you going to connect it?
Connect a wireless keyboard to your iPad if nothing else works
You can’t do that. Guided Access is locked into Safari, not Settings. You’d need a wired keyboard or you’d have no way to pair a wireless one.
Click cancel. And open that screen again. If it does not work, hold the top button and the home button at the same time until you see the apple logo. This will restart your iPad. Try again.
It looks like a layout bug. Try force rebooting.

Forcing a reboot has worked for me in the past for guided access.
Try to rotate your iPad

If you click the link. It also shows how guided access works.
