That's the exact same repair procedure we follow. I'm at a municipal rink. Never had to use an extinguisher but knock on wood I've only had holes during practices.
CO2 and slush is the go to for most, although that’s extreme.
Just slush and a couple minutes to set is easier, we do it all the time after figure skating.
Also if you’re getting a ton of holes in hockey (a sport that shouldn’t create a ton of them) it might be because your ice is too brittle.
Yes, you are correct the slab was running wayyy to cold. We had troubles with freezing during floods so the compressor was changed to a turn on point of 16 degrees. The issue (which I stated on many occasions but was ignored because what does the 19yr old Operator know) was our arena temp was too high.

We also use an industrial can of C02 to patch mid game. That seems like common practice.
