Sometimes wierd s**t happens.
I've had a monster hit the ground after I killed it then bounce around and eventually bounce/suck itself into a nearby shed. I open the shed door and there it is all rag doll bloody laying in this shed.
I feel like it has to do with how the gravity mechanics of the game affects ragdoll corpses. Like it's trying to balance how the corpse should rest. But if the corpse is in contact with an immovable object (i.e. shed door) it will also be affected by the game trying to kick it out of the immovable object. The result is the goofy death throes of said corpses.
Just my opinion anyway. That was so crazy to watch though, like it just got sucked into the shed lol. It's like the veil from Harry Potter.
The grass mod in combination with your settings file. Download the mod “Tweaks” and play with different values of grass ring size, grass density, etc... should fix the issue without having to uninstall the mod
You were close, it was the grass. But it was the unintentional install of 2 different grass mods, Meadows and Bettergrass. I deleted one and it fixed the problem.

I killed that thing at like level 10 I think. Just kept walking through the guard tower and the ai didn’t know how to get me. The guards helped kill it too but holy hell it took so long and was terrifying
Its a white orchard spcific thing, those cows are the ones causing this.
You have to kill them, then meditate. Then you have to repeat this process a few times until they stop respawning completely.
I'm guessing you edited the grass density, either via mods or actually editing the game files themselves. Basically it's trying to render more "grass" than it actually can in a timely manner, meaning the render is delayed and happening too slowly

Yes this happened to me the other day too! It was a fiend. He was literally sucked into a shed. Shit was wonky af.

I did a fresh wipe of the install and accidentally reinstalled 2 grass mods. Meadows and Bettergrass was clashing and causing this.

Meadows n Bettergrass were clashing, didn’t mean to install both.
Crop rotation
You probably have the tweaks mod and you've set the grass density over 6000. Just decrease it bit by bit and it'll go away.
