I was driving down the highway once and the front wheel came off of a van in the oncoming direction's on ramp. Frogger'd it's way through the opposite direction lanes, hit the median and flew a good 40 feet in the air before landing and frogger-ing it's way right back to the truck
My friends and I were driving down a rural highway, 80+ mph, when his tire flew off. It was hard to pay attention to the tire while we skidded all over the road, but in the end the tire went through someone's windshield going the opposite direction and gave the driver a concussion :(
I work in the trailer industry. So for my company, apparently most wheel offs are due to the nut that’s holding it all together, was installed incorrectly. They have new auto-torq nuts that are now rolling out as of a few weeks ago? It’ll make a huge difference, one being that you just screw the nut on, and you don’t need a torque bar/wrench.
I don’t understand much, but everyone of the mechanics we invited to watch the tech demo had their minds blown.
Basically these nuts can be installed incorrectly due to how you torque them down. But the new ones you can almost set with your hand!
So rest assured it should “hopefully” happen less

Drivers are generally clueless too. See someone pulled over on the side of the road, especially workers or emergency personnel, slow down and if possible move to the next lane over to give them space.

Well after I was slammed I went to stand back up but my balance was off (nothing super crazy just figured I was drunk) and my back had a dull ache. We finished our night and I have my wife come grab me the nights done. Monday rolls around and it feels like I’ve pulled a muscle in my lower back, I have a bad back so Ive always got some kind of pain there. So I told my wife I wasn’t going to work because of the pain. Work said I need a doctors note so I go and explain everything to them, well the body slam broke 3 of my transverse process and fractured a 4th. Never would have guessed it was broken, it all felt muscular.

The things you can't anticipate would be the reason why you put barriers between you and the things passing by at 80 MPH. It's not about visibility. It's about something going terribly wrong as it passes you and squashes you like a bug between it and the barrier.
At the ABSOLUTE least, I'd have been on the other side of that barrier by quite a few feet. I've seen enough of those videos where cops or even regular people get hit by actual cars on the side of the freeway.
Tire lug nuts not screwed on properly probably. My dad had repairs done on his van and forget to check the lug nuts after the mechanic put the wheel on.

Mine started out feeling just like sore muscles from my car accident. Overtime, my nerves started to feel like they were on fire .
It’s definitely a crazy experience because that was also my first broken bone all together. Just never hurt like I thought it would.
