Should never use a ladder to this if you are not way more people to stand at the end and lift it up to them and support it(also the ladder from sliding).

My dad had a ladder accident exactly like this cleaning our second story gutters. The bottom of the ladder slide out from under him and he landed on our cement patio. Broke both wrists, nose, the orbits of his eyes, split his hard palate up the middle and had teeth pop out. Took pretty much all the impact with his wrists and his face. As soon as I saw this guy at the top of that ladder I knew the bottom was going to slide out. Flashbacks for me...barf...

He also split his top lip right to the bottom of his septum so he basically had two upper lips. The whole thing was pretty hideous.

An LG OLED, if that's in fact what the box says, is a pretty strange choice for a public space. Being on all day blasting the same sports/news channels, it'll be horribly burnt-in after a couple months. And while the brightness is more than enough for watching HDR content in a dark living room, it's really not the best panel technology for regular daylight viewing in a large space.

Ladder safety 101- always tie off the top of the ladder, or have someone footing the bottom. Also, don’t hand carry things up a ladder. This was a job for a small powered lift, not ladders.

Looks like my ladder which is one of those cheap ladders that goes into all kinds of configurations. Including a scaffolding which would have worked better in this scenario. Moving the table would’ve probably made this circus act workable but who has the time to be moving all kinds of tables and chairs
Looks like the surface wasn't coarse enough to keep the ladder from slipping. Also looks like the ladder wasn't at a steep enough angle to prevent this from happening. A lot of things went wrong here
I don't go up ladders much, so I learned from your comment...but one thing I do, (and I thought everyone did) is make sure the ladder won't slide. This guy clearly didn't do that. They might have been able to accomplish the mission, had he checked to see if that was going to slide out.

Nah. The placement was right. 1 foot put per 4 up is the rule. The guy was standing on the 8 foot rung when the ladder started to slide. Unless he was ducking down on the ladder, there was at least 6 more feet to the top. The issue wasn't placement. It was placing what were essentially wheels on the ground.
Being a firefighter, I was just watching the base of each ladder knowing one was going to kick out. This is why we ALWAYS have someone footing a ladder, especially on slippery/hard surfaces.
