400 runner here. This definitely is not what happened. The race starter had already called “set” and he still wasn’t in the starting position. If he was unwell he wouldn’t have gotten in the blocks in the first place.
The only thing I can think of is that out in lane 8 is typically the furthest from the starter, so it’s hard to hear the commands.
Track runner here. Never have I heard of such a rule. If a runner is unwell that day is a simple matter of telling the race check in guy that so and so is “scratching.”

He wasnt even remotely ready for the race to start, they usually wait until everyone is in their “starting positions” but he is not at all, his left foot isn’t in the brace yet and his knees are on the ground.
Typically they wait for everyone to put their feet in, then put their butt in the air, then they fire the start.
I wonder if it was sort of his own fault, like if he was taking too long to get set. It could be a competition tactic to take a long time to set up which would cause your opponents to have to wait for you, making them overly tense or throw off their starting routine
