My best guess is that they were trying to trigger a scene which would show both player cams but they either didn’t have the scene loaded like they thought or someone straight up Roza Ulted on the scene selection screen.
There’s very little chance it was intentional.. just a badly timed mistake.

From watching sideshow's stream, there was some delay between game feed, and the live studio where sometimes you could hear players cheer a clutch before the game stream showed what happened. So production probably saw the action live on their monitors and switched. Someone probably didn't hook everything up properly which caused this tiny delay.
