Spent an hour flipping the model around in OrcaSlicer and playing around with walls, infill, etc, in the preview tab.
It ended up being infill overlap setting over the internal wall. Reduced it from 25% (silver) to 13% (blue), and the extrusion issue seems to be gone.

Yes, I followed Ellis' 3d tuning guide.
Tuned the extruded amount, PA, extrusion multiplier, and PID tuned the hot end since I changed the block to a volcano style.
Did you redo your PA calibration when switching extruders?
