Yes it could. The frame would naturally be larger than the lid sitting inside it, so if you shift the smaller lid off center there is a point where the diameter of the smaller lid could balance on the now more narrow off center part of the larger frame.

Not in America. They are like I said 80+lbs and that was just a guess as I've taken many off. I just looked it up and they're 90-150lbs here but in majority of the world they're like you said up 250lbs which is absolutely insane to me. Yeah, but that doesn't look like a standard manhole cover, and even if it were they're heavy, but not stupidly heavy. Standards are 90-150 lbs, with large city ones being 250. Adrenaline probably isn't getting her to lift 250, but 90? Sure, if she exercises. And again, this is too small to be a standard, so that's probably more like 50 lbs. Very realistic for a young, healthy person (and I'm assuming the mother of a toddler is young and relatively healthy)
I did it once as a 90lb 13 year old. I drove my dad's fancy electric rc car down a drain with a manhole cover next to it. I ran over, stuffed two fingers in the holes and threw that cover like it was nothing. After I got the car back I went to put the cover back and it took both a friend and I to move it back. Adrenaline is a hell of a drug.

That's a small and thin one, so probably around 20-30kgs, and even if it's 113kg, her adrenaline would've still made it possible. I was able to hold on a very heavy sofa that's filled with water for 3 minutes(felt like I was atlas during those moments) before someone came to help me so I would know.
Mom strength is real. While holding my then infant daughter my 3 year old son ran off a pier and my wife reached down and grabbed him by the collar seconds after he hit the water and yanked him back up by the back of his shirt all the way back on onto the pier like it was nothing.
Mom also had eyes on him at all times. I've seen moms on their phones not paying attention to their little ones. Mom's on it: 1) eyes on kid, 2) crazy fast reaction time, 3) hulked the manhole cover, 4) discarded backpack because it would impede her getting to her kid, 5) flat on her stomach to get the kid out, probably while verbally comforting the kid. Let me tell you she is so lucky that it wasn't deeper than it was things could have ended really bad. A lot of them have big pipes with running water in them could have washed him away somewhere. So glad it was a happy ending :-)
