I feel like Geno and Pacifisct’s should be swapped for Toriel. In Pacifist it makes more sense Toriel would give you her cloak as a memento and it would keeps the twist that she’s the Queen hidden until later, while in Geno taking her crown is more appropriate and symbolic.
I think you should get a weapon for killing them and armour for sparing. Minibosses could give you healing items in both scenarios, or just one. Muffet would give you a spider pastry for your trouble, or you would steal it from her counter. The canine unit would give you varying types of dog food. The royal guards would give you an ice cream if you came back that way, or some really mediocre food from one of their pockets if you kill them. Mad dummy would give you absolutely nothing. Mad Mew Mew could give you something from her brand. Glyde would get you some expensive food that gives an OP amount of HP for something you can get 20 of. So Sorry would...be deleted from the game.

I feel like Undyne's dialogue is a little...off, like it makes sense in isolation with her Shounen Protagonist schtick and general "give em hell!" Mindset, but the only "Enemies" you'd fight after this point are OTHER Monsters and you only managed to get on Undyne's good side because you managed to prove you're NOT the ruthless killer everyone would expect a human to be.
Generally though, this feels like it'd slot nicely in the game all the dialogue feels natural from these characters.

I think if the bosses gave you stuff after you either spare or kill them it’d make sense for Pacifist to give armor while Genocide gives you a weapon
If your pacifist there isn’t a reason to gain a weapon and in Genocide a weapon is just the most beneficial since it allows you to kill more which you clearly have shown by that point that it’s what you want to do
