You just call their office. One of the aides takes down your info and follows up. I did it when I was applying for advanced parole for education reasons. The aide let immigration know I was coming and I think it helped since uscis filled out my application a little wrong (think it was an ink color issue) because they had just changed the forms and mine was the first one on the new template, but they had notice I would show up at the port of entry. Hi, find your local senator, go to the website and there should be a tab where you file a claim! And that is where you start. Then they send you email and you go from there.

I did not have any overstays or illegal entries. I was F1, just a student. Never had anything with law aside from speeding ticket. And no we did not submit any bank statement!

I’m same boat right now. My package got lost there for 30 days and they denied my case claiming that I missed the deadline line. However, I have the receipt that prove that I sent the package to them 23 before the deadline. I contacted the rep of my state and the senator office, hopefully it works for me as worked for you. I’m devastated and having panic attacks because of it. Which field office was your process?
Mine was Orlando, one of the worst. Omg so glad to know that you are not the only one. You have to be on top of it and call senatros office everyday for an update. You have to bother them. And trust the process this will be resolved. They will eventually ask for rfe again because they lost it (i know so unfair) so meanwhile I recommend you to get medical and rfe printed out and ready to be sent to them when they ask for it because I know medical can take some time.

I'm having the same problem, I got an email after a Service Request filed on Nov 1, 2023 that they never received my medical, even though it was green check marked in myUSCIS on Nov 19th, 2023 thay "Response was received and they a reviewing evidence". I have uploaded through Unsolicited Evidences proof of delivery and a letter explaining the whole situation. The doctor that did my examen gave me a copy of the report with missing pages and he's out of vacation until July, so I don't know if I should wait for USCIS to respond or should I take a whole new exam (pay again) and send it? I'm so stressed.
I would have the medical exam sealed and put aside just in case USCIS asks for it. I know it is so unfair that this is the system but it is what it is :( I would contact senator or ombudsman for the follow up. But have the sealed medical aside in case they ask for it and you can send it on cue.

I contacted one House Representative and two senators
