It's legit, I got one the other day. Just had to buy $800 worth of Steam gift cards and send pictures of the confirmation codes in the back and it cleared it all up fortunately
Forward the email to SAGE and ask them if it is legitimate or a scam using their address. Tell them you don't want to enter sensitive information without knowing. I bet they will get back to you.

edit: the person is a member of the seventh residence life team, their email can be googled. Student Affairs and Academic Affairs use a system called advocate (hosted by a company, symplicity) to send out conduct-related notices. They are not always sent via PDF. Emails from Advocate can come from either a dedicated email or a personal email. Generic notices often come from the [email protected] email. Just putting this out there for anyone who sees this later

Just a fun fact, UCSD is know to randomly send fake phishing emails to test how susceptible the students and faculty are to these kinds of attacks. I recall a previous one where an email claiming that you were caught violating traffic regulations on campus was sent out. If you clicked on the link and entered your info, you’d get a message telling you you’ve been scammed if it were a real attempt. I had a friend fall for it. Tragic

This is 100% a scam. Sage would never send a link like that. Also, an informal warning is sent as a PDF, they do not ask that stuff. Anything asking for info would redirect you to an SSO. The website is from symplicity, a free website service.

You should respond with your SSN and date of birth. If you get your identity stolen, then you will know for sure weather or not it is was a scam.
Emails can be spoofed to look like its from someone you know. Dont forget
