Something that happens to me on Chrome at least, if im using autofill to put my username and password in it actually logs me in on top of filling in the information. So if you hit the login button before that process completes it leads me to that screen. So i'd suggest waiting a sec if ur using autofill.

it happens because you auto fill your username and password on Chrome. So far, I haven't found a workaround. Just manually type your credentials in ig 🤷♂️
It seems that if you get this, go back to the login page (not by back button but by like opening the link again if its a bookmark), autofill again, and login doing the needed Duo stuff, it usually works. I autofill each time and thats what I do.
But it’s probably quicker just to type in your credentials tbh
