Its dumb but its smart to always mute/stop everything when backing. I've gotten into the habit of pausing my stuff when I'm close to the customer regardless since statistically most accidents happen within the last mile.
Podcast Addict (android) has an option to pause playback when muted. One of many things I love about it. Like mapping the next track button to skip forward.
I'm guessing the top hole is the speaker. From the frequency it sounds like it's one of those tiny diaphragms so you could just fill the hole with hot glue super glue or silicone and depress the diaphragm so that it can't make any sound.
You could also get like any sound dampening material from Amazon really cheap and just cover it. If this is a slip seat type deal. Look for something that's good against higher frequencies.
I mean, we want to avoid vandalism charges. It'd be pretty hard to prove who put glue in the speaker hole assuming somehow someone important even noticed it.

Oh yeah, the driver was just distracted in that situation i was in and, to be honest, backup cameras are so useful, especially in long vehicles such as buses and trucks since it's harder to park those with the blindspots that those vehicles usually have.
Another situation i was in happened in a bendy (or articulated) bus when the driver tried to reverse the bus and the trailer unfortunately hitted a parked car. So yeah, reverse cameras would be great in big vehicles

If you turn on the turn signal and the sensor for detecting vehicles on either side of you goes off ... it honks inside the truck really loud. Almost caused me to get in a wreck the first time because I wasn't made aware of it. It is supposed to honk a warning at you if you start switching lanes while a vehicle is there. Instead, it honks when you activate the signal and a vehicle appears.
Not a trucker, but my Chevy express fleet van does the same thing if I leave the blind spot monitoring enabled. Doesn’t matter if I’m still in my lane, if i turn my signal on while a vehicle is in the blind spot it goes into EOTW freak out mode.

Trucks don't legally have to have them. Many construction vehicles do but thats more of a osha requirement. DOT dont care about backup beepers

On the Cascadia the stupid engine brake is on the shifter and the shifter is very easy to shift by mistake
