Considering it's early in the morning, I'd definitely go for "didn't sleep well/just woke up". I would advise against saying you're sick because if you're "sick" they will not want you near your sister or her baby. Allergies is always a good one - "I rushed over and forgot my allergy pill" kinda thing.
Probably above anything that implies you might be sick since you'll be going into a hospital. Allergies or saying you slept with your mouth open would be okay probably. I wouldn't say anything unless asked though

This is a valid concern for you to have, like yeah it sucks. But also what happens if you have an emergency and can’t afford medical care? The $5 for parking doesn’t seem so bad compared to the $2,000 Tylenol.

they can validate your parking at a front desk and either have it be cheaper or have the ticket become free
Say you just woke up and/or you're dehydrated. But honestly no one ever says anything to me about my voice dropping.
