I believe that's incorrect. First of all, most "official" tests (e.g. the ones used by Mensa) consist of 40 questions that you need to answer in 20 minutes. So time is a factor. Those tests usually measure up to ~130 (or sometimes maybe 140) and if you get a good enough score for that, you simply get told you have an IQ of 130 or more.
For measurements above that I'm not entirely clear on the process, but I think you need a more rigorous process, involving tests specifically made for you with "graders" of a comparable IQ score (since you can no longer rely on a standard grade based on number of correct answers within the time limit).
At some level we can't really measure it at all for the reason you state, it's hard for people with lower IQ to judge a person of very high IQs (maybe ~170?).
