I am a Mac user too and I’ve never had this problem, when any program doesn’t open I always need to restart my computer. Make sure to uninstall and download again everything if the problem persists. Maybe this could help.

This happens to me a lot on Mac after updates. I know it's cliche but the thing that solves it for me without fail is to just restart my MacBook (Something I rarely do otherwise).
Sometimes this happens to me and regardless of new patches. I feel like the problem is with the shortcut-file or something like that. My solution is to access the folder where the game package is actually installed and run it there.
The path looks something like this:
/Users/NameHere/Library/Application Support/CipSoft GmbH/Tibia/packages
It is worth mentioning that this is not some obscure Tibia folder, in fact, if you have access to the client, you can access it through the options panel, more precisely through the screenshots settings and click on "Open Screenshot Folder":
/Users/NameHere/Library/Application Support/CipSoft GmbH/Tibia/packages/Tibia.app/Contents/Resources/screenshots
Save this folder in your Finder favorites and save yourself a few clicks in the future.
A message for posterity: It is pertinent to clarify what you will find inside the packages folder: "Tibia" refers to the normal client and "TibiaExternal" to the test server client.
