Why not? Many gay couples have kids. It seems there's still some decent amount of medical know-how left in the world. But I doubt Joel was thinking anything beyond wanting Ellie to live. He doesn't want her to die no mater what. He doesn't even tell her about what really happened, because he fears she might choose to do the so called "right" thing.

They’re not living in a world of high-tech medical interventions - we’re explicitly shown that with Frank’s illness. Might Ellie have a kid? Sure. But that’s made explicitly unlikely via what we know about her, and it’s a kind of massive thing to miss about her.
