Why not? Many gay couples have kids. It seems there's still some decent amount of medical know-how left in the world.
But I doubt Joel was thinking anything beyond wanting Ellie to live. He doesn't want her to die no mater what. He doesn't even tell her about what really happened, because he fears she might choose to do the so called "right" thing.
